11/20/12, facility secured with fence, hatches secured, recirculating sand filter in good condition except for some small areas of wastewater surfacing, corrective action planned, need to remove low growing vegetation more frequently, ultraviolet light disinfection used in addition to fencing around drip field, need to plan replacement of corroded metal frame structures supporting pumps and UV unit in control building, telemetry system with call out / call in features, facility checked every 2 weeks at a minimum, records of site visits and chain of custody for samples maintained, need to add list of analysis parameters to chain of custody, drip field has some areas of incomplete fencing, corrective action was already planned, one area of surfacing wastewater / ponding, repairs will be scheduled, some valve box covers missing, will replace these, signs posted on all four sides of drip field, need to move one sign to access gate, no chemical violations of permit limits, facility meeting required frequency of analysis and MORs are submitted on time, operator certification requirement met.
