Cross connection program not complete. Has not been enforced or documented. Staff not following standard methods for daily lab testing. No daily logs. Calibration fluids out of date. CL 17 meter is not calibrated. Painting needed on pressure filter #1, rusty bolts need replacing, need screens on chlorine vent tubes, water pooling in finished water meter pit. Alarm in chlorine room not functioning properly. New line taps and repairs where pressure is lost are not being adequately disinfected or documented. Well 3 is leaking excessively during pumping. Not all records could be located and need to be re-organized. Customers not being notified of loss in water pressure/precautionary boil orders. Autodialing systems have had issues but have reportedly been recently corrected to contact appropriate personnel. Survey report sent as NOV.
10/24/16: received written plan of action response to NOV. Most items have been corrected and other items are going through Mason approval process before being corrected.
