Minor Deficiency Noted
Drainage from the road from the office to the asphalt plant discharges to area sinkholes.
At the final settling pond for outfall 001 the berm had washed out due to the inlet to the 8" DI outlet pipe being clogged with debris.
An emergency overflow point is needed.
Samples are collected by the plant manager & taken to the main office in LaVergne for pH measurement.
Samples are not always iced down.
The plant manager writes the plant # & date on the sample bottle & leaves it in the refrigerator at the main office.
No chain of custody forms or lab sheets were available at the quarry.
No records of daily O&G monitoring.
They have been collecting & reporting an annual SW sample from 001, it doesn't appear that this is required.
