Special Condition 1: Written notification of the commencement of authorized work was not provided to the Nashville Environmental Field Office per special condition number 4 of the general ARAP for construction or removal of minor road crossings. Special Condition 6: Road crossing #1 (35.980996, -87.091709) has altered the hydraulics of the stream by installing energy dissipation outlet structure to the downstream portion of the crossing (see photo 32). General Condition 1: The least impactful method of construction has not been employed for road crossing #2 (35.980464, -87.091917) – see photo 34. General Condition 2: the authorized activities for road crossing #2 have not been in conformance or consistent with information submitted in the ARAP application. General Condition 14: work being conducted in the flowing water has not been for a short duration and has caused impacts to the hydraulics of the stream and does not conform to methodology supported and endorsed by the Division. Photo 35 shows the utilization of erosion controls measures (wattles) within the stream obstructing flow along with accumulation of sediment on the downstream side of road crossing #2. General Condition 15: work on road crossing #2 has not be carried out in such a manner as it will prevent violations of water quality.
