Evidence of significant sediment discharge to and build-up in Hunting Camp Creek south of outlet structure. Sediment basin does not have a permanent pool and appears to have <50% treatment capacity. In addition, it contains several large rock outcrops within the basin with evidence of exposed soils with erosional rills and gullies. Basin area behind outlet structure has fresh covering of straw cover while areas on either side of the straw have rolled erosion control blanket with vegetation growing. Appears there may have been some type of problem with the basin's berm at this location. Twice weekly inspection report dated March 31, 2021 by Mr. Alain Ledgerwood indicates "pond outfall is under repair from breach, sediment in creek..." Outfall structure appears to have metal plates fastened onto the lower portions of the weir box. The riser pipe connected to the outfall structure is wrapped with some type of fabric. Twice weekly inspections are being conducted. However, a close review of these records from 03/15/21 through 04/12/21 revealed none of the reports had been signed by the primary permittee (Mr. Tony Cavender). The inspections have been conducted by Mr. Andy Travis Miles (certification # 144071 – expiration 12/31/23) and Mr. Alain Ledgerwood (certification # 137596 – expiration 12/31/23). The twice weekly inspection reports are incorrectly completed. Specifically, the reviewed inspection reports consistently indicate the phase 1 tracking number at the top of the form.
