6/24 & 25/10, operation & maintenance problems, plant deteriorating without proper maintenance, sewer rates do not appear to provide adequate funds to keep up the system, operating procedures not appropriate, no SOP manual, severe I/I & no organized sewer rehabilitation program, plant upsets & washouts, overflows at plant & in collection system, major deficiencies in self-monitoring program, selective sampling (days chosen) to get "best results" instead of representative sampling for CBOD, ammonia, TSS, & e. coli, EPA approved methods not followed for all analysis, records inadequate for self-monitoring analysis & sewer overflows.
