3/26/10, first day of PAI: severe problems with I/I, overflow ongoing at Wells Creek pump station & entering stream, not properly posted, not properly reported, no disinfection or cleanup of wastewater debris, no sewer rehabilitation program, much of treatment plant structures are severely corroded & not maintained, chlorine solution leak in chlorine room has eaten away the concrete block and concrete floor in one corner, vent fan not connected, metal wall panel & door still corroded & crumbling away, safety issues with chlorine room & rusted through hand rails over treatment units, only one of three blower motors operational, flow meters not calibrated annually, plant solids washing out due to hydraulic overloading, no storm mode operation used to deal with it, clarifiers have high solids levels & rising mats of solids, drag & skimmer on one clarifier out of service, little or no use of 2nd aeration basin, lab records have improved but still inadequate, no DMRs & many MORs not in plant files, new autoclave purchased, not following EPA approved methods of analysis for BOD & TSS. 3/31/10, continued PAI, not following EPA methods of analysis for D.O., pH, e. coli; much of self-monitoring data invalid; Wells Crk. pump station manhole still overflowing.
