WTP personnel were not collecting samples that were not representative of discharge effluent and no records of the sample collection were made. pH was being reported from the raw water of the WTP. Discharge was being estimated and the lagoons were being run with both valves open all the time.
On October 12, 2016 I performed a limited follow-up survey Ashland city. All of the items mentioned in the November 2015 survey had been addressed and fixed. Sampling is now being done in a location representative of effluent, pH is being run with each sample, and the lagoon valves are kept closed until a discharge is needed.
