Field staff attended final construction inspection of additional treatment facility, numerous deficiencies as provided below:
1. Fields are not fenced, which is required since disinfection is not in use.
2. Ponding water is already visible on the surface of newly installed drip fields.
3. Lines appear to have been installed off-contour, leading to ponding water in low-lying zones due to water flow when pressure decreases at the end of a dosing cycle.
4. Proper setback from property lines may not have been met in zone 1.
5. Water of unknown origin was present coming from under the Bioclere units and flowing off the property.
6. Installed new and existing infrastructure does not match approved plans. Drip zones that were not present are shown on the plans as “installed”.
7. Drip lines were installed in fill material in Zone 28 on the submitted plans—however, this is considered to be zone 6 by Adenus. Ponded water was present before and after installation of lines and should have been re-evaluated as stated in the previous site visit.
8. The system was turned on without Division approval or proper fencing. Ponded effluent poses a threat to public health and was not reported to the Division per permit requirements.
9. The location of the BioClere units, recirculation tanks, and EQ tanks were not installed per the approved plans.
