The NOV is for failure to submit the required technical evaluation of the need to revise local limits, failure to submit the required industrial waste survey, failure to enforce the pretreatment program, and failure to perform inspections at the frequency required by the approved pretreatment program.
The letter also requires the CA to submit the revised ERP and Form 4 of the most recent SAR. Minor permit deficiencies noted. It is strongly recommended that Mr. Smith attend formal pretreatment training.
