Permittee has not been estimating flow on DMR's, was instructed to do so moving forward. Some stormwater runoff enters a settling pond that is not covered in permit, including at next application could allow for increased acreage. Several windows in berms noticed, will be corrected. Spoils pile not adequately covered.
Site plan labels a channel as "Stream 1," stream has no buffer zone, has been blasted around, and a rock fall has cut off continuity. Josh Frazier will investigate records to determine if it has been previously determined to be a stream.
