The complaint inspection was the result of a fish kill reported by Mr. Ahearn. Chattanooga personnel were not available due to a training session and could not visit the site. Cookeville personnel Oakley Hall visited the application location and met. Ms. Karen Styers on site. The farm owner was also met on site. Synagro operator Ryan Respond was met on site. A notice of violation was issued by BSU.
