1/31/12, effluent samples not representative / taken at the wrong point, concrete sides of lagoons have broken concrete slabs & erosion under concrete & need structural evaluation then repairs, complete permit not on hand, DMRs / daily worksheets / flow records on file for 3 years, analysis & calibration records inadequate, staff not calibrating meters on each date of analysis, service company does calibrate twice per year, not using proper TCl2 calibration method, not following analysis procedure for total chlorine residual, outfall & sign not checked for 6 months, only single faced outfall sign, not reporting Fe results / preprinted DMR doesn't include Fe analysis, Fe may not be required because iron removal is not part of plant treatment, DMRs show no violations of permit effluent limits.
