This permit became effective on November 1, 2020, and will expire on October 31, 2025. The time period covered by this inspection is from December 2022 to September 2024. Whites Creek received a NOV due to two consecutive WET tests failing the same parameter listed above during the months of February and March 2024. Mr. Freeman provided an official letter on April 11, 2024, indicating an official biomonitoring investigation would be initiated. This included a schedule for the required Toxicity Identification Evaluations, Toxicity Reduction Evaluations, and successive progress updates to the Division. After internal process reviews, split samples were sent to two separate contract laboratories, which yielded different results; supporting the likelihood that both WET failures were due to laboratory error.
Trace amounts of rags, personal hygiene items, and other solid materials were observed within the influent distribution channel, primary clarifiers, and aeration basins but were not observed within the remainder of the treatment system nor did it appear to impact overall treatment. The refuse described are from the permitted septic haulers the facility receives. MWS will be installing an upgraded receiving station for haulers which will contain preliminary treatment allowing for the containment of refuse entering the facility’s treatment systems. Lapses in sampler maintenance documentation were observed. Recommended all maintenance and calibrations be documented.
