Evidence of sediment discharge to Hunting Camp Creek from phase (photograph 26). Evidence of non-structural EPSC failure; undercutting of silt fence (photograph 26). Evidence of sediment discharge to and build-up on the downstream side of road crossing #2 (35.980549, -87.091858). EPSCs have been installed within the onsite water feature (photograph 35). Unauthorized water feature fill located at approximately 35.981023, -87.091749 (photograph 31). Non-compliance found associated with NR2004.113 road crossing – see inspection report for more details. Twice weekly inspections are being conducted. However, a closer review of these records from 03/15/21 through 04/12/21 revealed none of the reports had been signed by the primary permittee (Mr. Tony Cavender). The inspections have been conducted by Mr. Andy Travis Miles (certification # 144071 – expiration 12/31/23) and Mr. Alain Ledgerwood (certification # 137596 – expiration 12/31/23). The twice weekly inspection reports are incorrectly completed. Specifically, the inspection reports consistently indicate the phase 1 tracking number at the top of the form. However, within the “Comment Section” both phase 1 and phase 2 observations and notes appear. Based on the observations documented on the inspection forms reviewed, the EPSC plan sheets onsite did not have any updates or modifications to the plan (see §3.4.1.b. of the CGP).
