copy of the permit was on hand, and the facility only retains physical records of effluent data dating back to December 2022. Mr. Roberts properly notified the Division within the 24-hour timeframe regarding effluent non-compliance with the WWTF in addition to subsequent updates on corrective actions and measures taken to prevent similar effluent violations. Mr. Roberts informed the Division that the series of E. coli violations were comprised of high flows due to a leak from the Facility’s fire pump along with the mixer for the chlorine contact chamber becoming blocked during this time.
The aerated lagoon appeared to be in good condition with no dead spots within the treatment area. No seeps or subsidence within the lagoon’s embankment were observed during the inspection. Cattails and duckweed were observed within the lagoon.
Outfall 001 was observed during this inspection. A double-sided sign with the correct information was properly located at the outfall. The effluent entering the receiving stream had no noticeable sheen or discoloration. Mr. Cassetty informed the Division that Facility staff inspects the outfall monthly but does not document the inspections. These inspections must be documented. Throughout the site review, the Division inspected the septic tanks. The lids were properly placed but unsecured. Given the septic tanks' locations near common areas and pathways, the lids should be secured to prevent unauthorized access.
