Notes
Inspection made with Kevin Blackburn and Micah Sullivan of the City of Fairview. Following observations were made:
1) Stabilization efforts made on sediment basin, but not complete. Structural failure observed on NE side in unstabilized area with slumping towards stream;
2) Head of stream at spring house not protected with effective and functioning EPSCs. Sediment observed in stream;
3) Sediment basin weir is wrapped in fabric, but fabric is breached. PVC riser is capped and rock filter is installed. Inside of the weir, the outlet is placed at the bottom allowing the pond to completely drain instead of establishing a permanent pool and sediment capture zone;
4) Rip rap placed at sediment basin outlet. Discharges across access road then to stream. Access road is eroding. Tree roots exposed in stream bank. Rock exposed in stream bank covered with silt from outfall. Silt fence along sediment basin breached. Silt and sediment build up observed downstream in creek;
5) Old weir observed approx. 30' SW of sediment basin. Discharging untreated stormwater to Hunting Camp Creek.
6) Filters placed in stream drains along roadway. Need maintenance soon.
