2/4/14, new two lagoon treatment system completed (previously had permit but no treatment for wastewater generated), 1st lagoon put into use April 2012, unpermitted discharge from sedimentation basins eliminated by combining with filter backwash (permitted outfall line) & both routed to lagoons, 1st lagoon hasn't filled to discharge level yet, lagoons are unlined & soil is porous / unsuitable, interior embankments are unstable & eroding, filter backwash line gets hydraulicly overloaded & overflows occur each time from manhole before entering the lagoon partly due to 90 degree turn in line, erosion of area around manhole, deposits of sediment from that & from wastewater overflow found in downslope & hollow below plant, engineering plans on site not stamped approved but approval letter on file, sampling of discharge didn't begin until Oct. 2010, numerous significant violations of effluent limits for all parameters from 2010 until April 2012 when wastewater was routed to lagoon, no exceedances acknowledged on DMRs, records retention good, working on MUR for self-monitoring program, need to develop O & M manual for system & plans for solids / sludge disposal method.
