Violations include removing duplicate disinfection, not testing all known backflow prevention devices, failure to report highest turbidities collected, failure to report major breakdown, Failing to meet the requirements of a directors order, failure to perform adequate maintenance of tanks and pumps, failure to perform adequate disinfection and bacteriological sampling for new and repaired mains. A date for a compliance review meeting was set.
On 16-DEC-15 I traveled to the Ashland City WTP and verified that duplicate disinfection had been installed. This installation met the requirements of the survey letter.
