Insufficiently treated stormwater has resulted in discharge of sediment into Brushy Fork, erosion prevention and sediment control (EPSC) measures were not been properly installed and were missing/absent, riparian buffer zones were not properly maintained and stream alterations were conducted without an aquatic resources alteration permit.
