1/5/2024 - RWR - Complaint Synopsis
Well owner (Martha Ryal) hired an unlicensed individual (John Quantos (sp)) to perform work on her well.
John Quantos – Company = “Paint Works” – 1700 Stallings Rd. Humboldt, TN
He worked on several parts of the well/pump system. Notably, he buried the well-head improperly and complainant mentioned that well owner had some trouble with brown water.
Complainant mentioned that the unlicensed installer also mows her grass for her and has been spraying round-up around the well-head and cover.
Complainant likely has the evidence necessary to proceed with an NOV.
Cummings Well Service (A legitimate/licensed individual) was hired to resolve the problems.
Complainant was advised that I would send him the link to the web-page where he can track the complaint status once I received an email.
1/8/2024 - RWR - received documentation (receipts, invoices, photographs) of the work performed at the well-head.
1/8/2024 - WNL - Will Little completed site visit on 1/8/2024. Surface Inspection passed. Well is presently in compliance. Photo Log uploaded.
1/9/2024 (RWR) - Program personnel to speak with licensed individual who performed the repair work before proceeding.
1/9/2024 - WNL - Will Little contacted Cummings Well Service by phone. As far as Cummings could tell - no work was performed on the pumping system. Work was done between the well and the pump system (Located in the garage). Well seal was broke and casing cut below ground.
1/10/2024 - Notice of Violation issued to responsible party.
1/30/2024 - WNL- Certified mail is being returned. Sent NOV back out by mail, uncertified.
