    Description and Writeup

    Mr. Dane Cutshaw, DWR reported Ms. Connie Bernard's complaint of sewage in her yard and that the alarm does not work on the individual pump station serving her residence. She has contacted the Town of Baileyton and said the Town will not do anything. SKV contacted Mr. Danny Neely, Baileyton WWTP who said the pump station was checked at approximately 9:00-9:30 AM this morning by Baileyton personnel and they found nothing wrong. They will check the pump station again and call SKV. SKV contacted Ms. Bernard who said sewage backed into her residence two weeks ago when the pump did not work and the Baileyton personnel did come out and worked on it. She also said the pump station's light still doesn't come on. SKV informed her that Mr. Jeff Abston, DWR would come by her residence today.

    After Baileyton personnel came by her residence, Ms. Bernard contacted SKV and reported that the light was replaced at the pump station. She also reported another sewage concern on the front side of her residence. She will take pictures and e-mail them to SKV.

    Mr. Neely contacted SKV and reported Baileyton personnel replaced the bulb for the red light at the pump station this afternoon. The pump in the station was checked and the top of it was seen in the station's tank. Also, the ground was dry. Mr. Neely also said Ms. Bernard was referencing the 1-16-15 overflow that occurred at her residence and she wants a new E-1 pump station. A written report will be sent to the Johnson City EFO.

    SKV received Ms. Bernard's two e-mails with pictures on 2-6-15. Ms. Bernard's first e-mail stated, "These are the pictures of the raw sewage that was reported 1-15-15 involving my tank." and the second e-mail stated, "These are picture I just took of the sewer leak that ran for 3 or 4 day that I am aware of. This is the front side of my home at 68 Jones St in Baileyton." Mr. Jeff Abston indicated from his 2-6-15 field activity report, "Received complaint from Connie Bernard , 68 Jones St., Baileyton, concerning malfunction of municipal sewer system resulting in effluent discharging from "holding tank" in back yard. There was no effluent discharging at time of site visit but it appears it has discharged previously due to an approximate 4' wide, 25' long swath of dark green grass starting at the tank and extending down slope....Ms. Bernard also pointed out an area across her driveway where recent excavation has occurred....There were puddles of muddy water around this area-Ms. Bernard contends that effluent surfaces in this area occasionally and runs down the street adjacent to her home."

    SKV and Baileyton personnel investigated Ms. Bernard's concerns at 68 Jones St. on 2-9-15. No sewage was overflowing from the pump station nor from the area across from her driveway. The pump and alarm light were seen in operation at the pump station. The creek downgradient from the pump station had clear flow and no evidence of sewage was observed. According to Baileyton personnel, a new pump was placed into the station during summer 2014. The inside of the station tank had rust and no holes. For the area across from her driveway, Baileyton personnel said the "blowoff valve" for the force main was replaced in 10-2014 and sewage did overflow into a dug hole beside the valve during the repair since there was no way to stop the flow. Ms. Bernard was informed via voice mail by SKV on 2-13-15 that the pump station was repaired in 10-14 and the force main in 1-15. Also she was informed that both overflows were reported to the Johnson City EFO per the Town of Baileyton's NPDES permit requirements.
