    Complaint Description and Writeup

    Complaint: I was boating on the Cumberland River yesterday & friends who live in the area are concerned by the Carlex (Former Ford) glass company’s discharge into the river behind their plant. They were afraid to go close near it. I thought it was some natural waterfall until they told me it was runoff from the plant (see hose stuck on cliff in the pic) I told them I would inquire. Is a NDPES in place? Have these discharges been inspected? By the way, there were a flock of vultures/black birds feeding on a small deer near that area near the bank. Not sure how it died. 5 photos attached (1-white residue on bank U/S of 002, 2-001, 3-002, 4-001, 5-Briley Pkwy bridge) ***
    On 6/15/15, MRT & ACT viewed the area of concern from the Cumberland River. The waterfall mentioned is the Carlex Glass 002 CW/SW outfall. The pipe coming down the bank in 2 photos is the 001 process water outfall. The white residue shown in 1 photo is not an outfall point. The white material appeared to start about 25 ft above the waterline. The material was dry & friable & there was no liquid flow associated with it. It was noted that there were no identifcation signs for outfalls 001 & 002 as required by the NPDES permit. ***
    On 6/19/15, MRT contacted Mike McWilliams, Carlex, & visited the facility. The area at the top of the bluff was checked. Outfall 002 is enclosed in a pipe up to a point at the top of the bluff. There was no evidence of any overland flow of material in the vicinity above the location of the white residue. MRT decended about 25' below the top of the bank to a point where there was a vertical drop. The area above that point was a dirt substrate & vegetation with no sign of any other material. Property owned by Ford (TN0080675) extends partially into the area above location of the white residue. The white residue appears to be a legacy seep & is not actively flowing. McWilliams said he had signs for both of these outfalls & would work on getting them in place.
