03/13/2024 - CAM - Was sent out to look at wellhead construction area and discharge of drill cuttings. Found the well manhole construction to be deficient. Site derived drill cuttings did appear to leave site. Well regulations do not afford oversight of drill cuttings.
04/08/2024 NOV is being drafted.
4/24/2024 - NOV mailed
4/25/2024 - violator is already aware of the NOV and has already completed the repairs to the well head - photos of repair uploaded to WL.
4/26/2024 - RWR - Uploaded NOV to WL - RWR to confirm with KEFO that issue is resolved before closing case.
5/8/2024 - CAM - Re-visited site to check manhole construction. Some repair work performed, but continues to not meet Standards. Low point of manhole bottom is even with well cap. Drain opening sits higher than said low point creating a pocket of water on the low side, unable to drain away.
6/14/2024 - CAM re-inspected - reports that repair is satisfactory - closing case.
