    Complaint Description and Writeup

    6/21/2023 - RWR - In application for licensure, applicant listed a well upon which he had installed a treatment device and a pump on 6/1/2023, after the Division had received confirmation from Mr. Parham would no longer perform unlicensed work. This is being treated as a second violation.
    8/7/2023 - RWR mailed NOV for second Violation - Will close case when letter is received by violator.
    8/10/2023 - RWR - Chris Parham has received NOV - indicated that the date on the application was a mistake. Assured that he has not continued to work since the original 4/10/2023 NOV was received.
    8/22/2023 - Board recommended to commissioner that the Division investigate this installation further.
    9/18/2023 - Email to Parham requesting documentation of the specific installation.
    9/18/2023- Received return email from Parham - no documentation of the installation. Closing case.
