    Description and Writeup

    Complainant called to report alleged miss conduct by water treatment plant personnel, including not disinfecting or sampling filters after acid washing them, not reporting highest turbidity on Monthly operation reports, not reporting a turbidity violation from March 5, 2013, putting turbidity meter instruments in "hold outputs", and running filters too long.

    01-OCT-14 site visit and investigation:
    A file review found that Harpeth Valley Utility District (HVUD) had already self reported the high turbidity reading from March 5, 2013. Review of the turbidity data at the WTP found that HVUD correctly reported the turbidity reading for that event, and there was no evidence the meter was put in hold outputs.

    Division staff had during the sanitary survey, which occurred on August 18 & 19, 2014, already found times when HVUD had not reported the highest turbidity reading on their MOR, this was addressed in the sanitary survey letter. None of these times represented a violation of the turbidity MCL.

    No evidence was found that filters were being run too long. Turbidity data indicate the filters were functioning properly.

    Review of the records associated with the acid washing of the filters did find that HVUD could only produce disinfection and sampling records for filters B-1 and B-2. All the other filters at the WTP were acid washed but no records could be produced for this. HVUD was cited in their sanitary survey letter for failure to make or maintain records for these events, and for failure to properly disinfect and sample the filters prior to placing them back into service.
