    Complaint Description and Writeup

    Mr. Mike Amyx, Holliston LLC reported an accidental discharge of solids from the cleaning of the outfall 001 discharge line on 4-11-16. The solids are contained on the right bank of the Holston River. American Environmental is going to use a vacuum truck to remove the solids from the bank. The solids will be placed in the Holliston's Wastewater Treatment Plant drying beds. No dead fish have been observed by Holliston personnel and no solids are flowing down the river. Mr. Amyx will submit a written report to the Johnson City EFO. The report received on 4-13-16 indicated, "Holliston was performing excavation work at the location of the 001 discharge line and noticed the discharge screen to be bent and leaning to one side. While attempting to adjust the position of the screen, pressure of solids build up behind the screen broke through, dumping settled solids in the discharge trench to the Holston River. The solids flow occurred for a period of approximately 10-20 minutes....Upon inspection, the discharge solids were noted to be contained to the river bank around the end of the 001 discharge pipe. American Environmental was contacted for emergency contract cleanup by vacuum truck to keep the solids from entering the river. American Environmental vacuumed solids from each side of the accumulation area on Monday until late in the evening. On Tuesday 4/12/16, American Environmental brought a 70-ft long reach excavator to complete cleaning of all solids from around the discharge area. All solids from excavation and vacuuming operations were hauled to Holliston's drying beds to dry, and are estimated to be approximately 40-50 cu yds. A new outfall screen was fabricated and installed, and cleanup of the deposited solids was then completed on 4/12/16."
