Site was active on the day of inspection.
Facility was working to construct sediment ponds in the northeast portion of the site on the day of inspection.
BMPs / erosion control measures have not been installed or maintained along the haul road and around the culvert crossings at the site. As a result, unpermitted discharges and sediment loss has occurred to the UNT of Colonel Fork Creek which runs through the eastern portion of the site.
BMPs / erosion control measures have not been installed or maintained to prevent sediment loss to the UNT of Colonel Fork Creek, which runs through the eastern portion of the site. Unpermitted discharge and sediment loss has occurred from the area where the sediment ponds are being constructed and entered into the UNT. In addition, there are areas south of the sediment ponds which have lost sediment to the UNT because no BMPs / erosion control measures were utilized.
Failure to maintain / install BMPs / erosion control measures has resulted in significant sediment loss in the northeast portion of the site. The sediment has traveled beyond the permit boundary and crossed the property line on the northeast portion of the site. The sediment has entered into a UNT to Colonel Fork Creek, which is located off the property and situated to the north of the site.
Failure to maintain / install BMPs / erosion control measures has resulted in sediment loss beyond the permit boundary in the northwest portion of the site.
