Notes
Inspection was conducted to follow-up on issues noted during a 2/8/2018 inspection by the KFO mining section staff.
A discharge from Outfall 001 was noted on the day of inspection. A discharge was also observed from Outfall 001 during the 2/8/18 inspection. This is considered an unpermitted discharge as the facility does not currently have NPDES permit coverage. The NPDES permit for facility is currently on draft.
During the 2/8/18 inspection it was noted the culvert crossing in the northeast portion of the site was impounded. During 4/24/18 inspection it was noted that the culvert crossing had been repaired. The culvert was replaced with what appeared to be a 20-24 inch culvert. The slopes around the culvert crossing were not stabilized. Additional stabilization of the slopes around the culvert crossing is needed.
During the 2/8/18 inspection it was noted that berms on the east side of the culvert crossing had been knocked down. During the 4/24/18 Inspection it was noted that the berms on the east side of the culvert crossing had been repaired. The outslopes of the berms were not stabilized. Additional stabilization of the berm outslopes around the culvert crossing is needed.
