This discharge, flowing from an AGC storm water outfall (SW1) into AFG Branch and then Madd Branch, was observed by BAE Systems personnel around 12:00 PM EST. BAE personnel indicated the discharge appeared to have ended by approximately 1:00 PM EST. BAE personnel notified AGC personnel of the discharge.Telephone conversation at ca. 2:00 PM EST with Mr. Art Green, AGC Flat Glass, revealed the discharge, consisting of glass fines and residual grinding/edging coolant, occurred during a routine tank cleanout when pumps failed to engage and pump the tank contents to a holding tank. AGC personnel had stopped the discharge by the time of the conversation with BBC. Mr. Green and Mr. Forrest Stratton, AGC Flat Glass, were advised that this release should have been reported under the provisions of the facility NPDES permit and that a written follow up report was required by the permit.During a 2/9/2012 conversation with Mr. Green, he indicated the discharge had occured because of the pump failure and subsequent overwhelming of secondary containment in the tank area. Overflow from the containment area entered the storm sewer. Mr. Green further indicated that interim measures have been implemented to prevent recurrence until equipment repairs can be completed and his belief that no residual material remained in the storm drain. A written report on the incident and corrective action plan is to be submitted within five days of initial report.Written report from AGC Flat Glass received 2/13/2012. The report erroneously indicates that AGC Flat Glass notified the division of the release. As noted above, BBC contacted Mr. Green of AGC Flat Glass after receiving notification of the release from BAE Systems personnel.
